The first thing you do if you want to make the next CoC is ignore all of those fags.
CoC and all of its copies are very wide in the fetishes they try to cover and all the copies that tried restricting that ended up with very limited relevance. Those games create a world and populate it with smut, if you want to copy it that's what you're trying to do: create a world and have the player take part in it. Get some decent gameplay mechanics (and don't have them just be a way to access the porn) and give the player a goal, even if blurry. Make characters you can interact with and write scenes for them and plan ahead to make it as easy as possible to add new characters, also start accepting works from other writers as soon as you reasonably can, but set some boundaries. You can't write a game the same size as TiTS on your own, at the very least you're gonna need some AI tools which will probably write nothing but slop.
Besides that, just don't hate your audience.

I just play AI dungeon. The free version is pretty good despite needing to wrangle it, and you have a lot of freedom in wrangling it (re-rolling outputs, editing outputs, and reminder instructions to the AI). I've never tried using it for video game elements like damage numbers but I don't miss it.

It's funny, there is a pig tf in game, but not a single pig npc. The Minotaur's have two different tfs for more or less cow versions. It probably should have been the same for the Orcs rather than just a random herb in the woods.
