Not exactly fat mechanics but if you make your character or your pawns fat in Dragon's Dogma they'll be harder to knock down by wind attacks and won't be carried away by flying enemies. There's other neat interactions like that but I can't recall any more right now.
what would an intuitive way to fatten up specific body parts be? thighs, belly, tits, etc
boobies can obviously be tied to milk products, for other parts i'm not so sure

Shadow Hearts: From the New World has Hilda capable of swtiching between slim and fat, which changes how she plays. Slim is a better attacker while fat is tankier and better at healing.

I think the best way to handle it would be by selecting or customizing a body shape/fat distribution in character creation in the first place, and then also some way to alter it in-game through magic or some shit.
Having stuff that affects exclusively one body part is kinda goofy.
