Haven't played the second game (yet) but that's how the first game was too, only like 5-10% of the game's content is lewd (10% if you include monstergirl-type stuff), the rest is all horror stuff.
As far as I know, the only major difference with the second game is that there's a "you" that can get involved instead of her just talking to the fourth wall. Also maybe some more transformations that have "actions" tied to them rather than just being transformations and spooky faces.
