Having almost 100% it, with only the last screen of the last level and the moon berry on my todo list, I'd say it can get very challenging. But for the most part it doesn't get to the absolute bullshit territory. By that I mean e.g. Super Meat Boy has those achievements where you have to beat whole areas without dying, and it can shove those up its ass.

Of the whole population, sure. If you hang out in enough game-related communities, you'll quickly realize that many if not all of the people you meet there are some flavor of autistic. This shit spreads like a wildfire in places with lots of autists. The ones who transition influence the autists around them who would be susceptible to also do it.
