Treat vampirism as a cult.
Gather followers to supply you with blood and keep your secrets.
You have to keep your true nature a secret from most of your followers, lest they flee and attempt to expose you. For this I think you need a more intimate view of the cult, and each individual member is his own character with his own story going on. The emphasis would have to be on your named followers, with some faceless ones too probably.
Of course, there would also be rival groups to oppose you. Law enforcement, enemy vampires, vampire hunters, media organizations.
Maybe the game ends when you've wiped out your vampiric rivals, maybe it ends when you've taken over the country, or world. Maybe it ends when whatever it is that made you has had enough and decides to unmake you. Or maybe you were yourself just a pawn in someone else's game.
This would probably work best as a strategy game, although I think it would need segments that allow your followers' individual personalities to shine.

ok so its a world where vampires are hunted to extinction almost and everyone is hyper aware of them
you are a vampire in a small town (200-400 people) and you only objective is to survive
now you can of course go up to someone and coerce them or just start biting peoples heads off but this will cause chaos and vampire hunter come and rape your ass
so the game is basically about stealth, planning and survival
you have to find ways to get a stable supply of blood without people getting suspicious so you can start building your vampire army
later on when people trust you enough you just open a blood bank but since everyone is aware of vampires it has extreme regulations and you basically have to sneak into and out of your own blood bank
also you can walk in the sun but you just weaker and look more tired during the day, your underlings CANT at all though

Could a True Blood vidya work? The setting is pretty kino and there are lots of different supernatural factions like werewolves, witches and others.

Id really like to see an RPG vampire game that has you go through time. You could start off in early medieval, Victorian, viking times or something. That could be a choice of where you start. The ends of chapters could be big events tied to a timeskip. Your choices could affect bloodlines of humans and the construction of civilizations depending on what choices you make. Tie mechanics to vampire powers getting stronger with how long you are undead, weakness to the sun getting more significant with age. Like when you first became a vampire the sun would only make you powerless and humanlike but after a couple hundred years it will burn your skin off and potentially kill you. By a thousand years you'll just straight up combust and turn to ash in minutes in the sun but your vampire powers are extremely strong by that point.

I'd like for it to play like a mix of cyberpunk/ the darkness gameplay but an in depth RPG akin to something like fallout new Vegas or kotor

The world shouldn't start with every area being patrolled at day and night instead it should be a reaction to your actions e.g. if you get caught pillaging a village. Npcs can scout and raid your castle with the castle itself allowing traps and shit to make it more interactive.

The old Vampire Redemption game had a timeskip from the medieval to the 90s. And it was kino.

Why not do this more than once? Hell, you could have it where the player POV comes in and out during time periods. Maybe after a torpor and wake up period. In WoD Vampires are mostly static and have bursts of activity where they start getting significantly more powerful, as I understand, so even sudden leveling narratively kind of works.

1 game with 2 options:

Option 1: Old Blood - Older vampire, much more powerful but recently awakened and playing catch up with speech and technology (mechanics) in the game world. Able to learn, but at a slower rate for the abilities. power-ups come much more readily.

Option 2: New-Blood - Younger vampire, recently made. As a result speech and tech come easier or are already known, powerups come more more slowly

Game: Generational War. The old bloods wish to re-assert dominance over the young bloods after the young bloods managed to gang up and kill the few old bloods that made them.
Young bloods wish to step out of the shadows and make their presence known to humanity.
The war is fought in 5v5 or 10v10 arenas based on real life cities. Young bloods can use / abuse tech and blend much easier with humanity. Old bloods are faster and stronger with double jumps / short flight.

Games are all fresh, think DOTA 2 or LoL - you can pick a character and play them for the match. Not a moba, more a brawl without guns. Also can have hide and seek game mode for olds looking for news. Or a hunt mode of several youngs hunting an old who has to hide / blend or fight back when safe to do so.

Another idea: Survival game set int he artic or antartic stations plays like 30 days of night. You get 2 ingame weeks to use the resources available to prepare after you get a message about the incoming problem from a nearby station / town. 10 vamps vs 20 players.
