Rumour is that Safia is an imposter. She's an ancient vampire who replaced the real Safia, who is found shriveled up in a coffin somewhere. Intuitively, fake Safia would have to be a Tzimisce using high level fleshcrafting. There's no proof of this, and it could just be schizo nonsense. But... it's kind of an obvious twist if you think about it. She's a little... too friendly.

Almost certainly. There's an Adam Jensen body in the Versalife Vault. There is a character you meet, a shopkeeper, who knows Adam, but Adam doesn't know him. His augmentations are weird. They're not the original ones.

It's also why the main menu is the outline of two Adams facing each other.

Also, some of the hate comes from the fact it is a very different game to the first one. It's a little but like Amnesia: A Machine for Pigs, except it's not a walking sim. Because the protagonist is an elder it's a very different sort of game tonally, mechanically, and so on.
