What would you guys do if you were somehow given total creative control over Minecraft?
(Remember that you have a budget of around 100+ million dollars per year.)

For me it would be:

Happiest time in my life was in 1st year of University, when I finished all my work early and was allowed to submit my exams much earlier than usual, and then had 4 months to do whatever I wanted. I had enough money to buy myself a treat each day from the supermarket, I had no responsibilities except washing myself, my clothes and bedsheets and keeping my space clean.
I got into the absolute best shape of my life, was the most relaxed i've ever been in my life and was happy and my living space was a tiny dorm bedroom and a shared kitchen.

If I had enough money to set this up for myself and never had to work again, I'd so in a heartbeat until I died.

One feature I always wanted was proper strongholds. Bigger and meaner. You add these intricate enchantment and potion systems, yet I barely get one reason to use them outside multiplayer PVP. Give me actual dungeons with dangerous mobs, clever traps, hidden levers, secret rooms, and chests full of loot. There are too many terrible additions in 1.21 for me to give trial chambers a try. There were adventure maps that did dungeons pretty well. Chocolate Quest has them as well, and with Recurrent Complex you can create your own.
