Depends on the genre. Rpg games are all about interaction so having a good amount of control on how your character is percieved and interacts with the world is important. Vampire the masqurade is probably the best example of this.

Boring. The mutant designs look sick as hell so if there's no mechanical downside there's literally no reason to not heavily invest in these skills. Kinda reminds me of the Illithid powers in BG3, you'd expect some kind of downside, but the only one is you become kinda fugly in act 3.

They do, I like Dragon's Dogma's changing encumbrance & resistance to winds based on your character height/weight. There should be stuff better suited for shorter, taller, heavier, lighter characters. Also making girls with explosive boobs and bodies built to pop.
