Practically? Not much. They come from the same genus. The red back is smaller and lives pretty much only around Australia, New Zealand, etc. The black widow is primarily in North and South America, but it is considered to have a global distribution. The venom of both is very similar, but black widow bites tend to come with muscle rigidity and cramping. Black widow webs are somewhat chaotic, while red back webs are more orderly looking. Red backs tend to be more aggressive and prone to biting than black widows.

Yeah, it's pretty nasty stuff. Luckily black widows are pretty shy and build their webs in out of the way places, and if you don't go sticking your hands directly in their webs or corner them you aren't likely to get bitten.
