The reason you die on higher floors is not the game throwing bigger threats at you, it's already escalated to the max around floor 21, but because you're fatigued

More challenge modes ala Dyle (Timed floors, itemless floors, etc.)
Changing maps like in L4D2, for example deeper floors could look more ruined and flooded by ichor, they could have areas that are inacessible and new ones that are revealed by collapsed walls and stuff
