I went as far as to try to shoot the pain using a slow motion speedhack on the exact same animation frame and he did not react despite doing so on the original game and the health bar not being grayed out, as in, the shot could damage him. hard is harder than normal. enemies take more shots, you take more damage, and you get half the bullets and throwables. if it's your first time, just do normal and do whatever you want. mgs 3 is best when you just absorb the story your first time, and your next playthroughs are harder difficulties, no kills, stuff like that. i've beaten the original a dozen times, it's one of those games you can play over and over again.

it's a mixed bag. hard makes changes to the stealth design that you might find unwelcome, more enemies, farther line of sight, areas were probably designed with normal originally in mind. on the other hand normal is just way too easy when it comes to encounters. you can pretty much mindlessly tank everything first try if you're halfway experienced with videogames in general, hard gives enemies and bosses just enough edge that you nominally have to engage with their mechanics. neither is perfect but I'd honestly say go with hard.
