It is part of the theme. That's pretty obvious with all the videos showcasing it. The bigger problem is if the game will be scary. Just looking at the monster designs it doesn't feel like it. It's more horror themed than psychological horror. This is the Spirit Halloween of Silent Hill or Homecoming 2.0 fro what they've shown so far.

Ryukishi was the writer, not the director. He's got very little input on how the game will present its horror elements. Reading the metro article linked in the OP the journalist who played the preview even says there's no reward for killing enemies and you can mostly just run past them, so you never have to really deal with threats. There's even a sanity meter but it depleting just seems to decrease your critical hits on enemies.
