I don't think it's explained in the game, but it's reasonable to assume that the technology developed based on his own DNA simply doesn't work on him or that it's somehow connected to the institute's experiments and is part of the reason why he's dying.

You don't need foreshadowing for for any of that when it's explained and fits in with the story. Synth Shaun exists because real Shaun was dying and wanted to live vicariously through him. That's why he tells him to go with you no matter the route. Kellogg is just a guineapig to the institute and got parts using Gen 3 synth technology to let him do his job better. Their experiments on him led them to create the coursers.
