The Nihil mechanic is cool even if you can heal through it. Forcing the player to have to save heals for an undodgable attack I think is pretty cool unless you have a very specific item.

All the endings of Elden Ring are attempts at making something better for the next Age, to varying degrees. Yes, even Dung Eater and Frenzy Flame, one is "if everything's shit no one is shit" and the other is ending suffering by ending everything.

But the Lord of Blood doesn't have a real philosophy that makes anything better. It would just be insane freaks attacking each other all the time. Scarlet Rot is the same, it would be it's a madhouse wherever that shit takes over. If you can't make a good enough argument for how it's better than the current shattered order than it won't get an ending.

I thought it was just Mogh recruiting Omens, not actually turning his followers into omens. Also, I feel like Age of Despair would be different from an Age brought on by the Formless Mother. In Age of Despair everyone becomes Omens where as a Lord of Blood ending would a "survival of the fittest" Age where the chosen and blessed are those who can spill the most blood.

Every ending can be presented as negative in some way with Fromsoft not so subtly hinting at Frenzied Flame being objectively evil. So again I don't think it's a good reason to not include other Outer God ages like Age of Rot or Blood as possible endings. The real reason we didn't get them is probably because Fromsoft just didn't have the time to include them.

He doesn't turn them into Omens, he binds them with himself through cursed blood, himself being the Lord of such Order, so they grow horns naturally, as beckoned by the Order their soul is bound to. You can collect the Seedbed curse off the flesh of Omen and create a mending rune that does the same effect. People will not be Omens because Omens are the results of wraiths without guidance accumulating through horns and tormenting the host. With the Age of Despair the Curse becomes a Blessing, and the cursed souls have somewhere to be conveyed and beckoned as they multiply and spread
