Apparently that's Wada Ryuji, and he's the jockey of TM Opera O, Meishou Doto’s rival. Now, Doto is usually a very calm and friendly horse, but here he was uncharacteristically aggressive.
There is a theory that tries to explain this: Doto repeatedly heard his trainer, jockey and others say “Opera O” with a sad face around him, since Doto would always get very close but lose to Opera O. Being an intelligent horse, Doto understood that “Opera O = bad thing that makes his humans sad”, but being just a horse, he never understood “Opera O = that fast horse he runs with a lot”.
The theory here is that Doto misunderstood that “Opera O = that bad human who sits on my fast friend, whips him and make my humans sad”, and so think of Wada Ryuji as an enemy.
Despite Uma Musume Doto's calm and collected depiction, her IRL counterpart was an aggressive horse when racing in his younger days and then eventually mellowed out as he got older
Tldr: Meishou Doto hates TM Opera O, but from his perspective, "TM Opera O" is the jockey, not the horse
