The game was originally way different, it was essentially going to be what Elden Ring is. A year or so before it was suppose to be finished they weren't even close to done, so they had to scrap a ton of shit and cobble it together what was complete the best they could.

areas were moved around and stitched in places they weren't supposed to be after the directors changed, majula to shaded woods and heide to no man's wharf have entire maps made later in development just to connect them
the canon explanation is that drangleic isn't 1:1 in-game like how pic related shows

When Dark Souls 1 and 3 breaks its world logic in obvious ways, it's almost always for the sake of emphasizing the otherworldlyness of a location (Ash Lake, Kiln of the First Flame, etc). There's nothing really special about Iron Keep, they just wanted a lava land for their DaS2 theme park and didn't really care about logical consistency when placing it.

Drangleic is literally a dream world (the game outright tells you this), why the fuck do people still give a shit about "inconsistent" world design in a dream?

Those aren't even obvious, you get to Ash Lake from a long trek down the hollow tree, you wouldn't notice it's not low enough for what you see from the Tomb of the Giants unless you're meticulously mapping out the world. The Kiln also has you get taken there by Frampt or the other guy so it's completely up in the air how far you went.
Iron Keep is just completely in your face blatant with how stupid it is, you would have to be completely oblivious to not notice something was off after getting out of the elevator.
