Those aren't even obvious, you get to Ash Lake from a long trek down the hollow tree, you wouldn't notice it's not low enough for what you see from the Tomb of the Giants unless you're meticulously mapping out the world. The Kiln also has you get taken there by Frampt or the other guy so it's completely up in the air how far you went.
Iron Keep is just completely in your face blatant with how stupid it is, you would have to be completely oblivious to not notice something was off after getting out of the elevator.
