I'm on NG++ and I'm still not sure what her friends beef is with her. She's being forced into a marriage against her will. One girl keeps calling her a traitor, the other is a jealous bitch. Her boyfriend is just weak-willed.

Lost in the Fog was fine up until Shimizu Residence and the parents fight, holy shit. I actually ran out of healing items in there
But the worst was the ending 2 final boss on that difficulty. Incredibly tanky.

Sakuko did die of a heart attacks and Rinko is a fire. Shun is not dead yet, he is confirmed to be at the wedding (another ending mentions that he is wearing a tie), but he'll kill himself if you go through with the wedding.
Also, but Kotoyuki and Hinako's mothers have cancer (Sachie and Kimie), this is proven later in the game.
