So let me get this straight:
Hinako was taking red capsules forged from the flower that the cult in the old games used to communicate with the Gods. This medicine also makes you hallucinate and go insane.
So the communication with Fox Mask in the spirit world was real, that was her communication with the Gods, but her visions of the town covered in fog were a hallucination manifested from her fears of what's going to happen when her father sells her.
Do I have that right? Interesting that if so there's no real "Silent Hill" like other world since the area that would most resemble it never actually existed.

The otherworld is actually in the Shimizu residence that you visit near the end of the game. After you switch out the calendars you pass through the real version of it, and the ones created in Hinako's mind representing different points in her life.

Personally I assumed that was more hallucination logic which was progressively making less and less sense as her sanity was pushed to the limit

Nah it has the classic Silent Hill otherworld red lighting we've come to associate with otherworlds, plus an entire room with a massive eye that references Silent Hill 4

Yeah sure, the game is excessive with it's use of the unreliable narrator trope, red herrings and fake out endings. I'm sure you can project whatever meaning you want on the game. I like to believe the whole game is about a Fox God and an old Sword God duking it out for dominance over the next generation of towns folk.
