I want to translate my horny weird desires into mechanics that make intuitive sense but I want to have some unique spin on them. So many games have done weight gain as "decisions of what meals you eat over time" or "how many portions you choose to eat".
Right now I'm thinking of a narrative experience like a detective or VN based on a mystery but I am struggling to incorporate weight gain and fat into it in a cohesive way.

The only way i've seen games made out of that fetish is stealth games, which always has the character try to avoid being seen, which is fine except i actually prefer it when the character does get seen, so theres no way to make any kind of game out of it while matching my tastes
In that case, the only thing i could possibly do is make a visual novel, but then there's the problem of getting assets and writing a narrative that actually lasts any susbtantial amount of time, and i have no idea what story to come up with
Even if i used stock assets for basically everything, i'd still need CGs and unique art for the characters, and i'm not an artist nor do i know one willing to work on something like this, and i dont wanna make koikatsu slop either
And even if all of that wasnt a problem, adult games are impossible to host anywhere these days because of payment processors

Because most fetishes don't really work as games. They are better as just art or comics. Yet I'm shocked by how these shitty VNs are able to milk a large audience. I've seen better appeal in doujins with the same plot. They are actually finished, have way better art, and I can read them for free. But for some reason people want shitty art with a scrolling textbox with sex scenes that never come and annoying control scheme that makes it hard to beat off to.

There's no point to producing entertainment software that would 100% get removed from the vast majority of the market, because it would have to stray directly into the territory of pornography to accomplish its thematic goals and world setting.

The only reason why something like OP's fetish game isn't deleted (or even restricted, apparently) is because there aren't any exposed genitals or female breasts. It's also one of the easier fetishes to ignorantly point at and say "wow that's actually very cartoon-logic humor, and definitely not someone's sexual fetish/fantasy!" and then nervously laugh about it.

Likewise Coffin of Andy and Leyley isn't about the suggested incest route, it's about cannibalism and murder, which is acceptable because most videogames are already about murder and involve eating/hunger. Also it's reduced to a single dream-sequence scene where both characters are shocked by the implication of their demon-induced vision; the actual act never happened on-screen, graphically.
Rimworld, likewise, would be ended because of the cannibalism and sex, but it's so abstracted and non-graphical it again gets away with it because nothing is actually seen.

Racing/Chase gameplay and hitman style "taunt this target, then escape" with a time attack mode for speed running.
The character(s) in question wear a full-body and skin tight "Slip suit" that has extreme abrasion resistance and a low friction coefficient, while letting sweat pass through. The suit in question however is totally transparent, revealing everything beneath it, so the character has to be painted or covered in a flimsy or washable coating to be considered non-nude.

The goal of the game, is to direct their character to the objective as quickly as possible while defeating other contestants in the race to do the same, however each time they slide or roll or climb something, they become more nude and attract more attention. Bonus points for taunting the crowds, or using phone calls to put a crowd in the area. Eventually the fun-police get called and directly chase any too-nude contestant, which punishes incompetence in the raceway route. Obviously the contestants can "politely" fight one another by removing each other's modesty coating. To literally "re-cover" the modesty coating, there would be construction sites or graffiti artists with paint cans (with sick designs for bonus profile/attention, at the cost of time loss) or if the character is sweaty they'd roll in the dirt, or cover themselves in (very fragile) balloons, or rip open a bag of cooking flour on themselves.

there is not and that says something because honestly a lot of the world building is just "cool thing the dev liked" and the writing is "joke the dev liked" but it's cohesive enough, it's actually finished, and most importantly, it's actually really fuckin' funny sometimes (the insistence on playing things straight and only using parody as loving homages, not "haha aren't we so silly?" borderlands shit, probably helps). on top of that, some other thoughts on my head:

I checked out Succubus in Wonderland recently and what immediately stuck out to me is how perfect that game's mechanics would be for a slightly more vore themed Dungeon Crawler RPG.
