When people began exploiting glitches in the game and finding ways to break past the limitations enforced by the code, a glitch was found in the "Zombie Horde" minigame that allowed the player to go out of bounds by getting blown up in a very specific way, essentially "riding" the explosion to the outside of the map.

There was a glitch that let you get out of bounds during a zombie survival mini-game which allowed you to explore an empty version of the game world. The glitch was real, but the urban legend of "The Freezer" is that if you explored in the empty city long enough, the game would eventually freeze with the shadowed silhouette of person being projected on the walls. Allegedly this could brick your copy of the game somehow too.

It's entirely unfeasible. Some early 360s had an issue where the disc reader would etch a big circular scratch into the disc under certain circumstances. I believe The Freezer destroying discs was probably inspired by reports of old 360s eating discs. Though the legend also says that it can destroy digital copies somehow too requiring you to buy a new digital copy, which doesn't make any sense.

There's been some investigations into the Perfect Dark Chicago Ghost urban legend, and some evidence has been found which suggests it may have been kinda real. The stories of the ghost typically go that the player does the trick to get into the normally inaccessible Pond Punk bar where they are then killed by an invisible enemy. Sometimes they find dead bodies of guards they didn't kill first, and sometimes the story says that the invisible enemy that killed them used a crossbow.
The "invisible enemy" part can actually be explained by a hidden NPC pathfinding mechanic. The NPCs all need pathfinding node paths to walk somewhere, and the Pond Punk bar lacks these except for right around the entrance. If an enemy is currently chasing the player and they can't pathfind to you, they are able to teleport to somewhere nearby which is out of sight of the camera. If you shoot at all inside the Pond Punk bar, it's able to alert enemies on the street which will then start circling around the entrance to the bar unable to reach you. If you go to leave the bar now, there's a very high chance that one of them will teleport into the bar directly behind you as you're leaving and shoot you in the back. If this kills you, it'll appear as if you got killed by an invisible enemy as you won't see the guard that killed you while you're dying.

There's also the very rare and still not understood "Janus Guard" glitch which might explain players finding dead bodies in levels. Sometimes it's possible for an enemy guard to randomly turn into an ally and start killing other enemy guards, as if they were shot with the psychosis gun. It's still not known how to reproduce this, but it has been recorded by some speedrunners. As for the claims that the invisible enemy is armed with a crossbow, this is not something explainable and is most likely people making stuff up.

The Freezer is a real glitch and can brick the game if it happens, that isn't fake. It isn't an intentional easter egg though.

If you go frame-by-frame in all the recorded "Freezer" encounters, you can very clearly see that it's just photoshopped in. The aliasing (or lack there of) doesn't match with the rest of the clip, it often doesn't actually match the angle of the wall it's supposed to be projected on and in cases where the camera is moving it doesn't move with the camera properly, and in some cases you can see "The Freezer" overlaps the game's UI.
