Some of these exist in some format, but I can't think of many that might have a cool narrative flavor to it. Closest I can think of would be games or mods with alternate starting locations with penalties, and Portrait of Ruin like anon said, which is ultimately kneecapping the level cap so your characters fall off in power at a given point or just never get stronger. Suppose game endings that delete your save also constitutes as a malus.
