Doing a evil run last is the proper way to do it.
Doing it first just feels weird on your second run through, it's like you are trying to make up for what you did.
Evil run last though, now that is a proper send off, you've had your fun with that world now it's time to fuck it up forever and discard it, and move on to the next game.

Mainly the purpose of doing a "normal" run first is that you'll appreciate the differences more and get two runs with a lot of novel content out of doing it that way. A normal run where everything is new inherently, and then a Durge run where all the new Dark Urge content is there to add stuff and recontextualize the story you've already seen. If you're going to play two or more times that order works better. That said, if you're only going to play the game once feel free to pick Durge right away. It's not like it "bad" for a new player.

You need to be prepared to be evil. Killing certain characters would break and lock some questlines or made their walk trough not very natural. It wasn't really a big problem, I mostly doesn't like characters who's trying to use me, usually it was bad guys, but I've killed Jaheira just because she was too cocky around me.

By making Balduran fucking gay and a mindflayer, I think it's safe to say that we don't have to take the game as canon; it's poorly written currentyear fanfiction. Imagine if instead BG3 had the ethos of the first two games and was developed without resorting to ideologue insanity.
