You have to beat the entire game, all 3 DLCs, and then a dungeon that's 1000 floors long where every enemy past floor 900 will one-shot you even with the best gear in the game to then have a hidden bossfight at floor 1000 that rewards you with a weapon that instakills any enemy in the game

But Unlimited Blade Work has its uses. At the very least for deleting day version of Cenx-insert and getting the last summon weapon. Also for the skullhead pet if you can't be bothered with getting ammo or anything r*nged. Although i got one for myself way back when ninja+reflect damage could pull you through. And, technically, nobody prevents you from getting one of the older versions and do the same thing then just load your character in whatever version you do play. And for farming towers and Moon Lord for building bricks in case you prefer to play fair.

Blade of the East in Fallout: New Vegas. It's one of the best melee weapons in the game but drops off of literally THE last enemy you fight in an NCR run, and House or Yes Man runs where you don't fight General Oliver yourself (and even if you do attack the NCR squad that comes in after you kill Lanius, they're way easier to kill then him)

Kos Parasite (the one that lets you become a tentacle monster with the craziest, sickest moveset Fromsoft ever designed). You get it for beating Orphan of Kos, who is not only the final boss of the DLC, but also widely considered to be the hardest boss in the whole game.

Probably Majora's Mask's greatest design flaw. Though I don't really know where else to put it. If you put the fairy sword as the reward for the third temple, the the fourth temple probably gets way too easy.
