The entire thing was automated, unless Canon mode automates it. The target itself doesn't need to be lied to, you just do basic introductions and Naoe has no idea who it is. She just makes a guess and the guess is right and the game outright shows you she's the Shinbakufu member even though you have no reason to believe she is yet.

How do you even expect this to work at all in actual gameplay? What you are describing is Assassin's Creed Liberation which required you to go to a specific area to even change your Persona and it was never worth being Slave or Lady because the former couldn't do anything without racking up notoriety and the latter couldn't parkour so you just stayed in Assassin with a permanent tier-1 wanted level. The same would happen here if such a system existed.

Again, Nightmare difficulty combat and stealth.
Nightmare combat means enemies kill you in about three hits as long as your equipment is within 3ish levels of the enemy you are fighting, otherwise you get two to three-tapped if you're 10ish levels off, and two-tapped due to a "Can't be one-shot" system past that.
Nightmare stealth means enemies see you on rooftops, snow and gravel make enemies notice you earlier, unless you are fully covered by darkness you are never hidden, enemies actively investigate any basic sounds they hear like you breaking torches or bamboo walls, they actively go to rooms where lights are extinguished to light them again, they notice you as you are crouch walking up to them to stab them, and so on. You can still abuse their AI by running around a corner and going to a roof, or just using roofs in general, and sometimes small patches of grass do this, but that's about as bad as it gets.
