I'm playing NG4 on Master Ninja right now and it's basically this. I'm going to see it through since I'm already 2/3's of the way and it honestly hasn't been that bad but I doubt I'm ever going to replay it on this difficulty for fun.

>>need to beat normal to unlock hard
Still the worst shit ever.
Once I beat Normal I will not feel like replaying again instantly, especially if it's a long game and it was a mindless slog. Most likely I already picked up several bad habits too, so the switch to Hard will feel like trying to rewire myself for something that might not even be the highest difficulty. Fuck it, don't do this.
>need to beat hard to unlock hardest
Ok sure, I can get behind this one. I got a decently challenging experience instantly and refined my approach through trial-and-error, so I might be curious about whether that will carry me through the highest difficulty.
Also typically this is what devs consider it to be an extreme challenge at the limits of player skill, I go in expecting to get my ass kicked until I don't, or stress the mechanics until the game bends over. I understand this might not represent the only intended game experience so it has any right to be treated as a fun extra challenge mode.

Not strictly impossible but Trails in the Sky 2nd on Nightmare has a fight in the prologue that you cannot beat without metagaming (you need to buy a ton of items that there's no indication you'll need later), saving specific auto revive accessories and getting very lucky RNG

The ones that change engagements mechanically. SoP gives bosses more moves, Pillars of eternity changes enemy setups while giving them more abilities, and in MMOs bosses may even have new phases.

Pic was related. Quake is easier on nightmare than hard. This is due to the attack pattern, enemies never stop attacking, but it makes their pattern more predictable. On other difficulties they don't attack as much, but there's more RNG in when they do.

Shit like this is only acceptable in two cases:
>game has very high skill ceiling so you'll be playing way better in a mode that asks you to be better
or (something that, as far as I know, has never happened, just due to how complex this idea would be)
>get to experience a different story since you can play with the knowledge of what you've experienced beforehand

I remember that the Persona 5 re-release had a bossfight that spawned in lower level enemies where you had to use a certain mechanic to beat it but if you kill the lower level enemies too quickly it fucks up you pulling the mechanic off making the boss basically impossible to beat so putting the game on the highest difficulty often made it easier

>even at the height of the AI boom atm?
That's what killed the marketing of "AI". None of them actually had very sophisticated systems, they just made it obvious what procedures they were falling (FEAR) or had progression-based behavioural unlocks (B&W). Now that nobody's impressed by hearing the words "complex AI", there's no reason to doll up your game mechanics to be something they're not. One could package reinforcement learning into their game- people have done experiments- but it's computationally expensive and generally not satisfying for a player when the computer's just learning how to make them go from zero to dead as efficiently as possible.
