Most anything put out by triple AAA publishers, just because they're usually tuned a bit too forgiving. Though, I'll drop it back to medium if it turns out the balancing is really clearly meant for a second play-through

None. I play in normal and if I die a couple times I switch to easy then very easy. Dying disrupts the flow of the game and ruins the atmosphere

This. The more mainstream the game is, the higher the difficulty you should pick. Niche games that are made for people who actually play games are more likely to have the standard difficulty option actually be a reasonable challenge rather than braindead easy.
all of them? i enjoy struggling to fight the game, learning the mechanics and "gitting gud"
besides normal is usually such a cakewalk i can kinda do anything and just autowin everything, its boring
I always play on normal because it's the intended difficulty. If it's too easy, I'll still fault the game. If it's too hard, that means I need to play better.

Games that allow you to pick the hardest difficulty from the get go. Normal difficulty otherwise because that's my only option at the moment.

i played ME3 on insanity for my first playthrough
my reasoning was ME2 was already too easy on normal so i figured what the hell, why not try out insanity
it was still too easy except for two fights which were complete bullshit: the citadel DLC final fight, and the earth mission final fight
