I've been thinking about Distortions and how they manifest and I think I've been wrong about how it's a sense of losing 'it' with it being what keeps you going. What if it's a form of falling into obsession with something so drastically that the only way you can continue is to focus all thoughts on it and nothing else? Heath was solely focused on Catherine, The Maestro was entirely focused on his idea of art and what the dots represented. Bongy was obsessed with chicken. The theory kind of falls apart when I consider Kim and the man's daughter in Distortion Detective. But that's what I'm thinking.

I personally havent read DD, only Leviathan so I'm basing everything mostly on LC snd Leviathan and interpreting it from that.
The Maestro's Distortion was primarily triggered by the Mirror technology rather than 'regular' Distortion triggered by despair.
It's hard to say what the 'base' is but we can look at the general feel of it.
The Maestro turned into a spotted lizard person with the ability to peek into other Mirror Worlds.
The main theme here is the many dots representing the many Mirror Worlds and the act of 'looking'.
After losing to our guide he distorts and turns into an Abnormality. A box with drills and a keyhole with an eye looking out.
The design is almost a complete opposite.
Instead of blatantly showcasing the dots on the skin, its hiding inside a metal box.
Instead of looking at others its primarily focusing on those that look at IT, focusing on attacking them.
Instead of a biological lizard its a big metal box with mechanical drills.

I'm pretty sure if Kim was allowed to turn into an Abnormality he'd turn into a pretty nice 'Friendship is magic' beneficial one.

Trains are also expensive to operate. You're spending a fortune to essentially pack away people for less than a minute of real world time. And then what? Put them on another train ride? They're probably insane and puddles of flesh now (unless you strapped them down beforehand), but how exactly does creating insane people help you make a profit? Are you going to let them go? Execute them?
You could make an argument about putting prisoners on trains to farm time off them to sell. Issue is, you can also just farm time off passengers, and they even pay you for the privilege of riding on your train, so you can double dip.

My impression is that it happens when your world view/way of life is challenged. At that point, you're confronted with two options. You can 'give up', 'surrender', basically run away from your issues, maybe telling yourself that it was impossible all along, or that it's better this way. This is the path to distortion, and also the direction that Carmen tries to push people towards, because she thinks it'll make them happier in the long run (There's a lot more I could probably write about Carmen's motivations, but that's a different topic).
The other option is to continue to resist, to continue to challenge despite the obstacle, to continue to try to push through even if it's difficult or painful. This is the manifestation of EGO.
Take Yan for instance. When confronted with the fact that he was still following the prescripts all along, despite his rebellion, he gave up. It was meaningless, it was impossible to defy the prescripts, which means it wasn't his fault. It means that all he has to do is listen to the prescripts, and stop thinking.
And then compare that to Xiao. She ended up losing her entire division to the Library in her quest. Granted, we only saw half of her conversation from Carmen, but Carmen seemed to be telling her that she should survive on her own, to run away and preserve her own life and not waste the sacrifices of her companions. Xiao retorts that giving up and running away would be tarnishing the sacrifice of her companions, and that she will continue to push on and preserve, even if alone.
