Ok so Dante revives the Sinners each time they die, but to do so he has to suffer all the pain that the Sinner's death caused upon them. So if someone sliced Heathcliff's neck off, Dante would have to feel the pain of having his neck sliced and choking on his own blood all at once.

Gregor gets lung cancer because the retard and can't stop smoking. He lives for 8 years but in the end he dies to the cancer. He's still a Sinner so it's Dante's job to revive him.
