There's no need to theorize about his identity, he's named in Babylonia.
Gil recognizes him as Utnapishtim (from the Epic of Gilgamesh) and he's named elsewhere in that chapter as Ziusudra. These are two different names for the same character, a mythic figure from Sumerian and Babylonian legend who survived the end of the world and was granted immortality as a reward for bringing the Gods to justice. The bell motif he has going on comes from that legend as well. He's a recurring figure in mythology all around the bronze age middle east and cameos in Gil's epic.

He was tasked with living forever and eternally keeping watch over the world, bearing witness to the death of every last living thing such that Capricious gods could never meddle in the lives of mortals again. They literally exposition dump this backstory in Babylonia and then call back to it when KH shows up at the end. He witnesses Tiamat as the last living thing in the (under)world and thus renders her mortal.

Azrael is a unique angel in islam (it's a muslim angel, in case that wasn't clear) in that he was promoted to the status of angel rather than being created as one. Obviously Fate's scenario is just conflating these two things, that Utnapishtim becoming immortal was him becoming Azrael and his title as "angel" of death isn't literal. Azrael itself is a title that just means "servant of god" and not actually a name.

He feels envy to Noah because he was capable of saving someone, so to me, he is probably an OC who was the nameless survivor of the first flood event but unlike other deluge heroes, he was never capable of saving someone, but rather became a gatekeeper of the death.

Kinda the issue with it, there's a difference between concept and reality. Writing in his character sheet that he has conditions for his NP(which fundamentally don't exist since he still uses his spear against Atlanta at the end of Apocrypha), is different from the reality we see whenever he shows up and is just a generic cool guy who always has the same emotions. He's a very static character; like even with his heel pierced he shows the same static emotions.
