Just marathoned this game for the first time since my childhood (I never managed to beat it back then). Really amazing game considering how simple and primitive it is.
Especially surprised by the controls, it's somewhat similar to SMB but better, you have breddy gud control in the air which you largely lack in SMB. Weakness is that if you land and after jumping are still holding the run button and the direction you immediately start speeding away. Died a few times because of this but ironically it also saved me once. Eventually I got used to it by carefully letting go of the buttons right before landing.
The platforming I'd say is better than SMB, you clearly notice the similarity but it's like they tried to make it more elaborate and interesting, with good variety between moving platforms or constrained spaces (which are underused in SMB). No water levels except the shmup part is fine, I never much cared for the SMB water levels (this improved in SMB3 and SMW but still not my fav).
Also it's quite short obv, compare to SML2 or SML3, but I guess they were just learning how to program for Gameboy. It doesn't feel unsatisfactory despite the short length. The shmup levels are absolute kino and add awesome variety in the game that doesn't feel out of place.
Other than the short length I much prefer SML to SML2. Probably equal to SML3 but very different in style and excepting the short length of SML1. Music amazing too and creative setting makes it feel much bigger than it is.
