At the time, Native American stuff was in-fashion so to speak. That's probably the inspiration behind wanting a naked Indian chick in the game but, of course, they had to come up with some plot and gameplay.

Believe it or not, erotica back in the 70's and 80's, and even before then, had lots of rape...Like, LOTS of rape. Rape was considered "erotic". And it's why most women's romance novels revolve around big, strong, dangerous, mysterious men taking them and ravishing them not wanting it, but simultaneously letting themselves be taken.

Women view romantic rape as the man showing confidence despite the woman's self-assuredness. The manual (which is only one page) has literally no story in it at all.
The idea that she's "tied to the cactus" and the idea of it being a rape game are just some headcanon people went with.
It doesn't make a whole lot of sense given you're trying to make your way to her with people shooting at you. It would mean Custer tied her up, then left for just long enough that people would notice and start shooting at him on the way back but not long enough to just go untie her.

Alright since it's not really a thing anymore I'll give you an example. This game is basically the equivalent of how people would watch the Catalina Collection pornos (oh shit I'm sorry meme) these games were pretty much exclusively something you brought out when you and the guys were sitting around drinking some beers and bullshitting. They were comedic and never meant to be taken seriously and, hopefully, no one was actually turned on by these games. They are some of the most absurd video games ever made and this goes for other ones like beat'em and eat'em.

Now for the actual porno "games" that people probably did whack off to, those were mostly for personal computers, Fuller Brush Salesman on the Apple II was pretty infamous, but there was plenty of them and they all were pretty terrible but they definitely were not absurd or comedic as the Atari games.
