Fast ROM is an expansion, "slow ROM" just means the stock SNES capabilities, which is 2.68 mhz speed. The fast ROM expansion uses special chips to overclock to 3.58 mhz, which is still half the speed of the Mega Drive. Other expansions that came out later, like the SA-1 or Super FX, did finally allow the system to be faster than a stock Mega Drive, but those were far more expensive cartridges.

Yes it is. Fast ROM games use special ROM chips that allow the CPU to overclock. They were less common, and cost more than stock games. You just want to carve out a special exception for them because the stock CPU speed of 2.68 mhz is so sad and pathetic.

Mega Drive versions of multiplat titles are almost categorically better, this is thanks the far better hardware. Hyperstone Heist is a different game, but it is smoother, has better controls and hit detection than TMNT4.

The CPU is designed to run at FastROM speeds, cartridge-permitting. It's much like how the Game Boy Color's CPU could run at a higher clock speed, but only if the cartridge was rated for it.

The CPU basically has to wait longer because slowROM delivers data more slowly. It's literally waiting for code to come from the slow ass ROM chip

It's just faster ROM chips. No extra CPUs, nothing even as extensive as NES mappers. The SNES isn't "overclocked" by FastROM carts; rather they allow the SNES to operate at its full speed.

Experiencing slowdown on SNES is honestly an extremely rare thing. If you just think about Super Metroid, Donkey Kong Country, Yoshi's Island, you just picture a seamless 60FPS experience with no interruptions. I think the only famous instances of slowdown are maybe the underwater sections of Mario World and the graveyard part of EarthBound.
