Adaptive resistance, like in Etrian Odyssey. Or go the SaGa route and make bosses so strong that spamming stuns and absolute guards is a mandatory strategy.

Healing can miss in Final Fantasy Tactics. Only one I can think of off the top of my head.

Only case I've seen this is in badly-made rpg maker games that don't tag healing items/spells as "automatic hits".

Consuming a healing drink isn't easy when:
>People are shooting at you
>People are casting fireballs at you
>People are swinging swords on you
>People are casting lightning bolts at you
>It's dark and rainy in the midst of combat
>You're on death's door below 2% HP, hands shaking intensely
>People are casting ice spikes at you
>You're outnumbered by hostiles 5 to 1
