I think those are entirely different.
3 goes for a very grounded, mostly realistic look (for the time and overall style). No one's particularly attractive, but I don't think it's an issue for that game when no one's going for it. I don't think it's an issue in 1 either, or 4 where a lot of people also looked rather plain for the most part.
The more modern ones however feel stylized the way you usually get when a consulting firm was involved. It's obviously stylized, but in a way that makes it worse more often than not. It'd be better if they didn't even try, at least then I wouldn't think about it.

Civ 4 uses the .nif format for it's 3d models which is the same used by Bethesda games, so it should theoretically be possible to export the body mesh from some sexy NPC replacer mod for Skyrim and put it in Civ 4 for even sexier custom leader models, although the skeletons are probably different so the animations wouldn't work. And if the leader animations are pre-rendered it wouldn't be possible anyways.

I don't think you understand how bad the conditions were in cities before the turn of the 19th century. Rome was pretty much the safest, cleanest, and richest city ever built before 1900 (and incidentally had the world's first mass public welfare programme) and the proles were still far poorer and more diseased than the rural farmers. Cities prior to modern times only grew due to continued inward migration from rural areas. Disease and starvation were so rife that death rates essentially without exception vastly outstripped birth rates.
