You got a negative ticking war score because you didn't capture the war goal, because you picked the wrong casus belli.
Do you understand how wars work in CK3? Because it sounds like you thought they work like in EU4, where you pick what you take. In CK3, the outcome of wars is predetermined by the casus belli
Capital being a war goal also suggests your casus belli was either a tributary war or an artifact war, neither of which would have given you Italy even if you had won.

Is it just me or are there not many things to do in CK3? I modify whatever i want before unpausing, but there aren't many things to do. I'm trying to play densely but only find myself doing some standard stuff each playthrough which aren't a lot. I feel like Vic 3 has a shit ton to modify before unpausing and quite some depth in the mechanics of the game, like, it's more dynamic and "alive"

No, you're right. If you ignore the character activity shit (which is actually quite barebones, it's just clogged with WORDSWORDSWORDS text that you can and will skip over) CK3 is an incredibly barebones and limited GSG. Even more so than EUIV, which is already pretty bad in terms of limited simulation. It's a shame Paradox insists on every game having a separate set of mechanics where anything they aren't focusing on gets horribly simplified.
