Youtube can still run ads in "Not monetized" videos.

Not monetized only means, revenue not shared with the creator. If a small channel posts a video, youtube can run ads on them too even if the channel is not on the youtube partner program.

It could also be the yellow sign of monetization, meaning the channel can run ads but the CPM is so low, the video cannot make any money for the creator.

I ran YT channels for years. The truth is that youtube doesn't care about the loli (particularly), but everything is automated without human input.
If a channel gets a big amount of false reports (from antis), it automatically gets flagged and the adrevenue dies.

No staff at youtube cares to check if the reports are valid because there is thousands of channels, they will keeping making money anyways. And most of the reviews are handled by AI or brown people in 3rd countries who don't speak english, don't understand context, and don't care too much either.

Sometimes this is avoided by having a partner at youtube, a real human being to talk to, which is something I'm sure hololive had and corrected for their channels. Only really big channels or huge companies get this privilege. The rest of the peasants have to keep guessing and self censoring forever.
