While nowadays Holos are okay, the early ones were really bad, even with improving L2D tech and the IR sensors. 3D is much more performance intense than 2D, makes no sense to have it be the default for most streams. Not to mention that Hololive 2D and 3D models are fucking garbage anyway, that the talents prefer using fan versions of their models when they can lmao

3D looks awful unless it's fully rigged for events and stuff. Hell even then they still haven't even tried to deal with the awful clipping issues of hair, clothing and accessories. Never mind not giving big hair good looking physics. So no, 3D isn't always better.

Requires specialists.
But the price difference isn't that high when you're talking high quality or corpo level expenditure.
A professional outfit like Mofu workshop will bill you something like 8k USD for a high end 3D with all the bells and whistles.
