I've heard multiple answers to this over the years and it seems "it depends" basically. For Myth I recall hearing from Kiara, Mori and Ina etc that the basic gist of the design is what it is but you can request some personalizing tweaks to it (just not a total overhaul). iirc the spike pauldrons on mori's shoulders were her request. on the other I think Ina wanted to be more covered up and they didn't honor that because that's basically a different outfit. kiara basically said something along the lines of "we don't really get to choose much" about the design. then again that's all EN opinions and reflected the state of things like 5 years ago now at that.
in general cover has the designs made first and then assigns people to them later. so they wanted some dyke char all along, they just weren't sure who they would get to play the part. come to think of it nijiEN tried a vaguely similar idea with who was it, yugo? only more extreme since it was a girl playing a guy character in noctyx. and likewise, that didn't go over so well. almost, like it's a bad idea to bring in people not confident in themselves in general about this kind of thing.
