Johnny and River met as kids and dated/married later on.
Joey is his twin brother who did not meet River, and died after Johnny initially met River under the moonlight, resulting in PTSD medicine or whatever to fuck up Johnny's trauma of losing his brother, which consequentially erased his memories of his first meeting with River where he had ultra rizz.

The tragedy comes in part from Johnny's amnesia and River's autism.
