make sure you get the ship upgrade (ship armour i believe) off jacob asap, don't do his loyalty mission at all and then in the finale send him into the vents. ded.
if you want to get rid of the coalburner miranda do the same thing re: upgrade (think hers is pretty useless anyway iirc) and no loyalty mission. take her with you for the final fight against babby reaper. she ded.
if you have the various dlcs and have done the shadowbroker one and found out that kasumi is also a coalburner from reading the logs at the end (that peom about jacob) it's the same process - get upgrade asap (cant remember what hers is, might be pointless), no loyalty mission and then take her into the final fight as well.
there are other diceroll ways to make sure the two coalburners die (the swarm part towards the end sometimes works but not always) but this method works best.
obviously do everyone elses loyalty missions (if you really want to you can do mirandas loyalty mission after having done jacks and then side with jack when they have a bitchfight which makes miranda hate you - lose her loyalty) and assign them sensible tasks in the finale.
congrats, you now have the best ending where everyone has survived except jacob, miranda and kasumi.
perfect.
it's a shame about miranda, her arse in the original (not played the gay remake) was gorgeous but on top of being a coalburner she wanted to behaviour chip your brain as she tells you near the beginning so she's a cunt.
