Can you create something with no reference? I've always found it weird how most cosmic horror entities tend to have human/animal like features as of conceptualizing something that has no relation to us is nearly impossible. So how do you actually create a lovecraftian horror piece that is so alien that no one has ever thought of before.

Come on, man. Lovecraftian horrors already exist. You're not going to create a lovecraft horror free of its influence of lovecraftian horrors.
It's like trying to shoehorn in elves to your fantasy world but they're TOTALLY not the same elves seen in other fantasy worlds.

Well, the trick here would be to pull a convincing "Lovecraft effect" of the enity simply appearing in a rationalized form to the human mind. Think like a form of mental censorship for the incomprehensible, if you want to cover it in a literary fashion it might be best to go for outright contradictory appearance behind the surface level of some "tentacled monstrosity". Motivation will be trickier from a narrative perspective, in case your story even requires you to go that deeply. I could provide a few story hooks here ... a monothematic primary motivation completely outside human motivation, a "mindless" mode of action contradictory to any logic yet appearing still deliberate, a motivation that might be relatable to humans but in execution goes completely against any mutual understanding ...

You can but it's the human and animal features that make them unnerving. I've found this picture of Yog-Sothoth that doesn't have any and while it's not something I'd like to see floating through the night sky I'd be much more scared if I saw a mass of tentacles, eyes, and teeth instead.

Language and music are natural. Some animals have regional dialects to their langauge, and an appreciation for music is widely reported with even the ability to compose music being a thing in some species. It's no Bach, but it's still impressive.

You can't really test for that since we still can't hard define ego. We have seen fads and trends in many species. We've seen recently learned behavior spread, and even taught to those outside their groups, in many species.
We have no clue how far down it goes. The more we study them the more intelligent we find them to be. Or the less value we realize we should place on intelligence to begin with, since it's not something that is unique to us.
