I think it's plausible but it still annoys me. I think the thing that makes it not work for me is that it's not really changing how he actually acts, it's just making him be a faggot internally and the only people who have to suffer through it is the audience. If the degrading of his mental state due to stress from the sister-lust affected how he interacted with his family more in ways other than his internal monologue, I think it would feel less forced. It could cause him to strain his relationship with a family member, or something in his hobby, or maybe even just school.
