It's a dying art style in manga, mostly exists in porn/ecchi now (for obvious reason).
It seemed big because of how prominent Ikegami, Hojo, and Inoue were.

But even among "realist" manga artists only a few does proper realism. Hojo and Inoue, for example, still heavily stylize their art.

Kenji's art is so fucking good. I find like half of his stories boring as shit or just plain not that sexy, but I'll look through anything he writes just because he's so fucking talented.

Although his current series is absolutely GOAT tier.

Obviously. The question in the OP was why don't mangaka use it if they can. I'd say it's because even if it's technically impressive you need to have both a discerning eye and fondness for it. It doesn't really play into strengths of the medium, which is depicting things out of ordinary.

It is nicer to look at than the normal ones.
The reason why artists don't do it is because it takes too much time or they just can't. So most artists settle with something on the level of (peak) Murata, or all those "realistic style" seinen manga in the past like Zetman, Gantz, and so on.

He probably means something like Tatsumi Yoshihiro. Gekiga is a pretty wide category, ranging from him to Ikegami, because it just means a cinematic/cinema-like approach to manga sequences/paneling.
