Tomboy Naruto doesn't change much for his (her) character. Naruto is a troublemaking orphan dealing with severe loneliness, everyone fears him (her) because of the beast in his stomach (the beast in her womb).
Even the fanservics scenes don't change much, Naruto laughing at pervs by making up a jutsu that turns her into an older naked version of herself is very in-character and on-brand for that era of manga. The biggest changes to deal with are Sakura and Hinata, since Naruto wouldn't crush on Sakura (and Sakura's shallow crush on Sasuke would pale in comparison to Naruto's at the end of the series), but Sakura can easily be made into a rival, where she's jealous of how much attention Sasuke gives Naruto.
Hinata is more complex, her Naruto crush either disappears entirely or she gets genderflipped into a man. A male Hinata would probably make Neji seethe harder because Hinata would be seen as so useless his younger sister got named heir instead.
It would also justify Naruto's obsession with Sasuke, first love and all that, the first boy who protected her and cared for her, etc etc etc. She would refuse to give up and insist that loving boy is still in Shippuden Sasuke, total "I can fix him" material. And it would turn Sasuke's asexualness into a bottled up repressed oneitis for Naruto.
Sasuke, on the other hand, is much more difficult to turn into a woman without rewriting several parts of the series. I gave it a try but you hit a lot of roadblocks with his character, the perception of him others have, etc.
