It's still a bit vague even in the manga (at least the properly translated chapters I could keep up with), but it's implied how we're seeing her now is not only her cleaning up her act a bit since her literal brain damaged personality switch, but that she's also "seeing" herself as better than she used to. We get flashes of her seeing a more depressing version of herself in the mirror that causes her psychic, and physical, pain before she rejects it and returns to her current up beat attitude.
Given how other people react to her, it's pretty clear they all think she's not that great looking even for her size, but eventually her infectious personality causes them to lighten up and not treat her as poorly and perhaps begin to see her a bit more the same way she see's herself now.
It's basically a gigantic thesis argument for the positives of self confidence and being more gregarious with people (in a socially acceptable way, of course).
