I think the D rather than being a bloodline thing is something everyone in a certain group willingly added to their names in order to show that they all believed in something or opposed something. So when Cobra said that Lily signed her name "Nefartiti D Lily" indicating she also counted herself as one of the group, that's what drove Imu mad
