I'm to retarded to give you a step by step, but as with anything the internet is your friend (even despite google search results being dumpster diving at this point). I haven't done a huge amount of metal working, but i wouldn't be surprised if you could pull it off with just hand tools even. Only real challenge i can think of off the top of my head might be the loop at the top.
