You can't tell. Even you would act normally when talking to someone with Down Syndrome. They could never guess something is wrong with them. The only way to know if something is off with you is if you're at the supermarket and the kids lock their eyes onto you. They don't yet have the social aptitude to know it's disrespectful, so if you're odd/ugly they will stare at you even if you stare back. This is a good indication you're not quite normal.

You have to be properly ugly for people to really think about it. People think being utterly average and unremarkable counts as ugly, it doesn't. Ugly people aren't just ignored, they're treated with outright hostility. If you were that ugly it wouldn't be a secret.
