I'm from Italy. I like the American one IE the only true one. The butter and cheese one isn't even called that in Italy. It's good, I guess, but I like the American one better although I don't use plain cream for it since it's not that common in stores in Italy. I using Italian cooking cream which is similar to creme fraiche but not remotely tangy. It's just got the taste of sweet cream except it's thick.
My Alfredo:
Parmigiano, 50g +extra for topping
Butter, 50g
Panna da cucina, one 100g carton
Grate the cheese, knead it with the butter (reserving some for topping) and set aside. This is called mantecatura.
Warm the panna, whisk in the butter/cheese mixture until well integrated and this with a couple servings of fettuccine.
Top each serving with additional cheese, as desired. Also good is to grate a little dried truffle on top. Also parsley.

this is a good example of why I watch american foodtubers for italian food because they are easier to make and tastier
it seems dumb to use all butter and parmesan when the goal is a creamy sauce. also annoying to mix it with the pasta in a huge bowl
