It goes both ways. I live in a poor neighborhood and when I switched doctors to go to a place in a rich neighborhood the doctors actually listened to me and ordered proactive tests and treatments. The poor neighborhood doctor, I'd go to him with a physical problem, and he'd be like "when does it happen" and I'd explain the completely normal thing I should be able to do, and he just shrugged and was like "well you should do that less then, is there anything else?". New doctor, oh ok does that interfere with your daily activities (yes), in that case here's some acupuncture, and let's get you 8 physical therapy sessions and come back and see me in a month.
So I don't blame poor people for not bothering unless they're literally bleeding out.
